Stomach stapling, also known as vertical banded gastroplasty (VBG), is a surgical procedure designed to help individuals lose weight when other methods like diet, exercise, and medication haven't been successful. It's one of the earliest forms of bariatric surgery, a category of operations aimed at weight loss. The core idea behind stomach stapling is to reduce the size of the stomach, thereby limiting how much a person can eat and leading to a calorie deficit and subsequent weight loss.
During the procedure, a surgeon uses surgical staples to create a small pouch at the top of the stomach. This pouch is then separated from the rest of the stomach, leaving a small opening, typically about the size of a dime, to allow food to pass from the pouch into the larger stomach area. The original stomach stapling technique relied solely on staples. VBG evolved from this, incorporating a band along with staples to create a stronger and more defined separation between the stomach pouch and the remainder of the stomach. Unlike some other bariatric surgeries, stomach stapling doesn't involve removing any part of the stomach, meaning it could theoretically be reversed by removing the staples.
The effectiveness of stomach stapling lies in its restrictive nature. The newly created stomach pouch is very small, typically holding only about 1 to 2 ounces of food. This means you feel full very quickly, even after consuming a small amount of food. By feeling full sooner and eating less, your overall calorie intake significantly decreases. This reduction in calories is the primary driver for weight loss following the procedure. On average, patients could expect to lose about 50 percent of their excess body weight within a year of undergoing stomach stapling.
While stomach stapling was a popular choice for weight loss surgery about two decades ago, it has largely been phased out and is now rarely performed. This is primarily because more advanced and effective bariatric procedures have been developed. Surgeries like gastric sleeve and gastric bypass offer better and more consistent weight loss results, with many patients achieving closer to 80 percent excess body weight loss. Furthermore, these newer techniques generally come with a lower risk of complications and fewer side effects compared to stomach stapling.
A common scenario illustrating the limitations of stomach stapling involves a patient who has successfully lost weight but finds it challenging to maintain it. Over time, the stapled pouch and the opening between the stomach sections can stretch. This stretching allows the person to eat more before feeling full, potentially leading to weight regain. This is a significant drawback compared to newer procedures that are more durable in their restrictive effects.
Recovery from stomach stapling surgery typically involves a hospital stay of 1 to 3 days. Upon returning home, rest is paramount, and strenuous activities should be avoided for the first week or two. Most people can expect to return to work within 1 to 3 weeks, depending on the nature of their job. A gentle exercise program can usually be started about a month after surgery to support continued weight loss.
The initial post-surgery diet is very restrictive to allow the stomach to heal. For the first few days, you'll be on clear liquids. This progresses to thicker liquids like pudding, yogurt, milk, and protein shakes for the first one to two weeks. Following this, the diet shifts to pureed foods for about 3 to 5 weeks, gradually transitioning to soft foods. It generally takes about 6 weeks to fully recover and return to your normal daily routine. During this recovery period, many patients experience fatigue and low energy, largely due to the limited calorie intake. This usually improves as the diet progresses to more solid foods.
Gastric Sleeve Surgery: Similar to stomach stapling, gastric sleeve surgery is a restrictive procedure. However, instead of just stapling off a portion, a significant portion (about 75-80%) of the stomach is surgically removed. This leaves a banana-shaped stomach that can hold much less food, leading to reduced calorie intake and weight loss. It is considered more effective and has fewer long-term complications than stomach stapling.
Gastric Bypass Surgery: This procedure is both restrictive and malabsorptive. A small pouch is created from the stomach, similar to stomach stapling, but the small intestine is then rerouted to connect directly to this pouch. This bypasses a large portion of the stomach and the first part of the small intestine, reducing both the amount of food you can eat and the amount of calories and nutrients your body can absorb. Gastric bypass is generally considered more effective for significant weight loss and improving obesity-related health conditions compared to stomach stapling.
